Icon Scientific, Inc. provides highly technical customer support. We are available to answer any of your product or application questions. Most service issues are easily resolved through phone support. In the rare event field service is required, we have a highly qualified and trained field service representatives.

Service requests can be submitted on line anytime. Please provide detailed information about your issue and supply item number, name of equipment and serial numbers. Make sure to include your name, phone number, email and physical address. State whether you prefer phone support or field service.

This form must be included with any items returned to ICON Scientific. Inc. for evaluations or repair. ICON reserves the right to reject any suspicious packages or those with incomplete documentation. Please send product with this form, the decontamination form and a complete description of the problem to:

Packing Notice
Each instrument should be packed inside a plastic bag to protect it from contamination. Use an appropriate packing material so that at least 3” of packing material is between the instrument and the shipping box. If possible, the original packaging should be used. Accessories such as cables and tubing which are not required for the repair should be removed.
